As we get older, health care often involves more than one condition and more than one medicine. Dr. Le looks at the whole picture, not just one problem at a time, and works with you and your family on goals that matter to you.

What is a Medicare annual wellness visit?
A Medicare annual wellness visit is a yearly appointment to create or update a personal prevention plan. It usually includes a review of your health history and medications, basic measurements such as blood pressure and weight, a check for memory changes, and a plan for screenings.
It is different from a full physical exam. Medicare explains what is covered on its website, and our front desk can help you understand how your visit will be billed.
Why are medication reviews important?
Many older adults take several prescriptions, plus over-the-counter medicines and supplements. Some can interact with each other or cause side effects such as dizziness or confusion. Bring every medicine and supplement you take to your visit, and Dr. Le will check that each one is still needed and safe together.
How can I lower my risk of falling?
Falls are a common cause of injury in older adults, but many can be prevented. Dr. Le can check your balance and strength, review medicines that may cause dizziness, and talk about vision, footwear and home safety. Staying active with exercises that build strength and balance can also help.
Tell your doctor about any fall, even if you were not hurt. A fall, or a fear of falling, is a sign that something may be worth checking.
What if I notice changes in memory?
Some forgetfulness can be a normal part of aging, but memory changes can also be caused by treatable problems such as medication side effects, thyroid problems, vitamin deficiencies, poor sleep or depression. If you or a family member notice changes, such as getting lost in familiar places or trouble managing bills or medicines, bring it up.
Dr. Le can do a short memory screening, look for causes that can be treated, and help you and your family plan next steps.

Common questions about care for older adults
Do I need a geriatrician, or can a family doctor care for me?
Most older adults can get excellent care from a family medicine doctor. Family doctors are trained to manage the health of older adults, including multiple conditions and medications. If your care needs a geriatric specialist, Dr. Le will refer you.
What should I bring to a Medicare wellness visit?
Bring all of your medicine bottles or an up-to-date list, including vitamins and supplements, your Medicare and insurance cards, a list of your other doctors, and any questions. Bringing a family member or caregiver can also help.
Can family members come to my appointment?
Yes. Many patients find it helpful to bring a family member or caregiver to help share information, take notes and remember the plan. Let us know if you would like someone involved in your care.
